**RETURN THIS FORM ALONG WITH BLS INSTRUCTOR REGISTRATION FORM**
Students wishing to attend MUST have the proper Instructors Manuals for the Course they are wishing to teach. EMT-TA will provide Students with required Instructor Manuals as indicated on the Program Application Page, along with the A.H.A/s "Essentials" Instructors study guide and Access keys to the online training program, as required by the A.H.A. Instructors wishing to teach the "Healthcare Provider Courses" Are Required to have Both...BLS Healthcare Providers Instructors Manual and BLS Heartsaver CPR-AED-F/A Instructors Manual which is included in the program fee.
"Important Note:" EMT-TA Requires that manuals will be mailed to Instr. Candidates along with Online Access Key at least 5 days prior to classroom date. All candidates "must" bring their manuals with them on the day of their class room session along with their Essentials Certificate of Completion.
Break and snack will be provided for students along with pizza lunch. Students with special dietary needs should please bring your own lunch. Lunch break time is 30 minutes.
